Facts About Testosterone in Men

How Male Testosterone Works
Testosterone in men is produced in the Leydig cells in the testicles in response to a hormone called Luteinizing Hormone (LH). LH is produced in the pituitary gland, which acts like a hormone thermostat. As the pituitary senses the body’s need for testosterone, it releases LH to stimulate the testicles to produce testosterone in men. Low male testosteroneoccurs as a result of 1) the aging of the Leydig cells in the testicles and 2) as a result of a decline in the pituitary gland’s ability to produce LH. The result is a low testosterone in men Decline starts about age 27 or 28 and its effects, or symptoms of male menopause are usually noticeable by age 35. If you are experiencing symptoms, it is important to get tested for low testosterone.
Testing Male Testosterone Levels
Lab testing for male testosterone produces three numbers: total testosterone, free testosterone and sex hormone binding globulin (SHBG). Total testosterone is the number most commonly used and quoted by inexperienced doctors,(See Below Male Testosterone Chart By Age ). The problem with testing for total testosterone is that the majority of testosterone in any human body is bound to protein molecules by SHBG and is totally inactive in the body. Only testosterone that has become free testosterone is active in the body (see male testosterone treatment). The measurement of free testosterone is the key to testing and treatment.
Andropause (Male Menopause) and Male Testosterone
Most men start to feel the effects of low testosterone and andropause, as free testosterone levels decline to below 130pg. The doctors at reNEW replace testosterone to levels in men to at least 200pg and as high as 250 to 350pg of free testosterone – where most men seem to feel their best. The so-called “normal” range for free testosterone is 80pg to 350pg (Note: range may vary depending on the lab doing the testing). Important: Don’t let your doctor tell you “you have normal testosterone for your age”. If you test at the middle or lower end of the range, and more importantly, if you are having male menopause symptoms, you need to investigate male testosterone therapy.
Male Testosterone Decline
Though individual men differ, the following table of testosterone decline in men is based on over 13 years of doing hormone therapy. The numbers include both total and free testosterone. The more important number is the free testosterone. The total is included here because it is the number most men are familiar with, and it is the number that doctors who are not knowledgeable about male hormone testing and treatment tend to focus on.
