Testosterone Injection: Enanthate - Cypionate – (T) Injections

Testosterone injections simply mean testosterone is injected directly into the body with the purpose if supplementing and raising/converting low T levels to higher healthy levels.  The key to effective and safe testosterone delivery is 1) stay as close to natural (bio-identical testosterone*) as possible and 2) take testosterone in small frequent doses.

Testosterone starts to decline at about age 27 to 28.  Men will usually start feeling symptom at around age 35.  Some men will feel symptoms as early as age 30.  Symptom due to low male hormones is known as male menopause or also called andropause.

Symptoms of Low T Include:

  • Fatigue and general loss of energy and the desire to be active
  • Irritability
  • Depression and a loss of the zest for life
  • Decreased sex drive
  • Decreased Sexual function
  • Loss of metal sharpness
  • Inability to maintain or build muscle

Testosterone Enanthate Injection - Testosterone Cypionate Injection

Most commonly used injectable testosterone is enanthate and cypionate.  Enanthate is the preferred injectable testosterone because it does not cause water retention as is sometimes the case with cypionate.  Also, enanthate is very close to natural though technically not natural.  It deviates from natural in that it is suspended in oil so that it pools within the muscle and is time released as blood flows by it.

Self Administered Testosterone Injection Therapy

Testosterone can be given with self administered injections twice weekly with a small 25 gauge 5/8 needle.

Many doctors are still prescribing injections to be administered once every two or three weeks.  This is not a good protocol.  It puts the patient on a roller coaster of high T for several days after an injection and low T just before the next injection.  The high T period also causes elevated estrogen which can be dangerous in men.  Injections once a week are acceptable but still cause some highs and lows.

Free Testosterone and Male T Levels

The objective of any testosterone delivery is to raise the free testosterone level to somewhere in the area of 300pg.  Please note this is FREE T, not total T which is more commonly quoted when talking about T levels.   The majority of T in the body is bound to protein molecules floating around in the body doing nothing.  Only the testosterone that has been converted by the body to a free state is doing its testosterone thing in the body.  Therefore “free testosterone” is the key number.

Testosterone injections have the advantages of:


1.    Easy to use – twice each week
2.    The testosterone entering the body is a fixed known amount
3.    Injection dose can be easily adjusted to adjust T levels
4.    Minimizes conversion to the stronger form of T called DHT – sometimes as issue with topical cream
5.    Eliminates the danger of transference to family members – sometimes as issue with topical cream

Men using testosterone injections are sometimes concerned about travelling with needles and syringes.  This should not be a problem with a valid prescription.  The doctor can provide persons traveling internationally with a “letter of necessity” to put in their language as an extra precaution.

Side Effects: Potential Testosterone Injection Side Effects

If done correctly there should be little to no side effects.  When starting treatment, the body has to adjust to levels it has not has for many years.  In the course if this adjustment, men might feel a bit more impatient with their environment – a feeling most men can identify with from their teen years.  This may last for a few hours to as long as two days. Men who have very low T levels are started on a lower dose for 1 to 2 weeks to minimize this possilility. 

*Bio-identical T – If you were to place testosterone from your body and testosterone from a bio-identical source – usually from plants – under a comparison microscope, you would not be able to see a difference.
